Thank you for your willingness to participate. The following information describes the general purpose for the study “Information Seeking and the Internet” and highlights key information you may be interested in prior to participating. Once you’ve read the following information, please click the “I agree to participate” button at the bottom of the page. Clicking this button will be considered your agreement to participate and will take you to the first page of the study.

There are a couple of key things you should know about the study:

·    The study takes about 20 minutes to complete and should be completed in a single sitting.

·    Please complete the study in a location that is quiet and free from distraction.

·    You cannot go back during the study. Once you leave a page you will not be able to return to it.

The purpose of the study is to understand the factors related to use of the Internet to seek out information about breast cancer. During the study, you will be asked to complete a questionnaire about you experiences using the Internet and perceptions of breast cancer. Approximately 300 women will participate in this study. There are two requirements for your participation: (a) you must be female, (b) you must be 18 years of age or older. Although there are no risks or benefits associated with participating in the study or any financial compensation, I think you might enjoy it (and I would be happy to send you a copy of the general findings if you are interested).

Your participation is voluntary and you are free to discontinue participating at any time without any penalty or damage to your relationship with the University of Arizona. The survey is completely anonymous. You will not be asked to provide your name or any other information that would identify you.

The records of this study will be stored securely and kept private. At times when the data is not in use, it will be stored on a secure, password-protected website available only to the researcher.
